Scary Game Overs

Considering Halloween is this weekend, I thought I'd create a gaming thread related to fear. In particular, Game Overs that you may have been terrified of when encountering them.
 
For starters, we have this little number here from Metroid Prime 2: Echoes-

This one put me off playing for a good while after I first saw it. Samus screams, the screen goes all monochrome with jitter cam, what is assumed to be her heartbeat getting faster and faster, until...
BEEEEEEEEEEEEEEEEEEEEEEEP.
GAME OVER.
 
Post your own examples here. And for the best results, close your curtains, turn out the light and close the doors as you watch 'em.

Actually just saw that one too. I see it as somewhere between Metroid Prime and Metroid Prime 2's game overs in terms of scariness, methinks.
