Need coding help AGAIN

Either I am getting stupid in my old age, or PHP changed!

I am trying to add a header to an image gallery and can't make it separate and do right.

I am trying to add this header:

^^^^^ That code may be messed up ... I want it to go to http://myfolderz.com/index.php

To this gallery file:

OR BETTER YET ..... if I could add a "myfolderz.com HOME" left of the "HOME" on bottom!!!!!

LOL, can't even figure out that simple task!
